Homestead Verification Form
Individuals and married couples are limited to
one homestead standard deduction. Property owners in Dubois
County will receive an additional form with their property tax bills
this year if they are currently receiving the Homestead Deduction.
This pink form, the Homestead Verification Form, is to verify
that only one homestead deduction is filed, and will be mailed out with
2010, 2011, and 2012 tax bills. It is required to be filled out at
least once by January 1, 2013 for the Homestead Deduction to
be verified.
Individuals or
married couples claiming the deduction must complete this form to
verify eligibility and to provide identification numbers (the last five
digits of both the driver’s license and social security numbers), which
will be used to populate a secure homestead database and prevent
homestead fraud. The form is not required to be returned in person, but
must be returned to the Dubois County Auditor for the Homestead
Deduction to be verified. More information on the form can be

Immunizations - Health Dept.
The Dubois County Health Department will make vaccine
available to everyone, regardless of their income or insurance
coverage.
Effective
January 1, 2012, the Dubois County Health Department will
provide vaccine as follows:
Children
who have any form of Medicaid, who have no health insurance (uninsured)
or are underinsured will continue to receive vaccines at the Health
Department under the VFC program as they have in the past.
Children will be considered underinsured
only if their private insurance policy:
• does
not include any vaccine coverage or
• specific
vaccines are not covered under the policy or
• they have a yearly dollar cap in
vaccine coverage and that cap has
been exceeded
All children who have private health
insurance will be
considered fully insured even if their policy:
• requires a high deductible be met first
or
• has a co-pay required or
• provides percentage based coverage
(such as 80/20, 70/30 etc).
Private stock vaccine will be used for these patients. The
insurance/Medicaid/Medicare eligibility status of the patient will be
checked at the time of service. All insurance cards must be
provided at the time of service. If a patient is considered
fully insured and the insurance company is not contracted with VaxCare,
the patient will pay the costs for the vaccine prior to the service.
Families who have private insurance
must become well acquainted with the terms and provisions of the
child’s insurance program, especially in regards to vaccination
coverage.

their county of residence.

The recent trend of delaying or
skipping vaccines has put children across the country at risk for
diseases like whooping cough and measles. It is
important for children to be immunized on time, every
time. Because diseases still exist in
this county and can be imported from foreign countries, unimmunized
people are at great risk of getting sick. Children are most
vulnerable to diseases until they complete their primary series of
immunizations. Therefore, children begin to receive vaccines
at the earliest opportunity so that they will not become ill, or worse,
die from vaccine-preventable diseases.
It is the goal of the Dubois County
Health Department to protect all children from vaccine preventable
diseases by creating awareness of the critical need for timely
immunizations and by providing convenient access to immunization
services.
