Immunizations

The Dubois County Health Department Immunization Program operates in conjunction with the Indiana State Department of Health (ISDH). All immunizations are given according to the Center of Disease Control (CDC).  We have the ability to bill through private insurance, after eligibility check is complete. 
Children, who have Medicaid, do not have insurance coverage, have insurance without immunizations/preventive coverage to cover vaccinations, or are American Indian or Alaskan Native may receive VFC vaccine. The administration charge is $15.00 per vaccine.  

We offer Adult 317 vaccine for adults without insurance. The administration charge is $15.00 per vaccine.

Children under 18 years of age must be accompanied by a parent or legal guardian or another adult over 18 years of age with our Parent Permission Form (English), Parent Permission Form (Spanish) signed by the parent or legal guardian, along with a copy of the parents ID. ID will be checked of named person accompanying the child.  Updated insurance card is required at the time of appointment. If you cannot provide these at your appointment, it will need to be rescheduled.
