Individuals can request a certified certificate of birth or a certified death certificate from the Health Department. To receive a Birth Certificate, one primary or two secondary forms of ID are required (if you do not have a primary). 

To Request a Certified Birth or Death Certificate:

1.  IN PERSON
WALK-IN'S:  Office hours are 8:00 a.m. - 4:00 p.m. Monday-Friday.  The office is open over lunch hour for issuance.

2.  THROUGH THE MAIL
Either Birth Request or Death Request forms (above) can be downloaded, printed, filled out, and mailed to the Dubois County Health Department along with payment and proof of identification.

3.  ONLINE
To complete a request online, the Health Department has a partnership with VitalChek. NOTE:  An additional fee is charged by VitalChek for the online service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.  VitalChek requests can take up to two weeks to process.

4. THIRD PARTY AUTHORIZATION
An adult may authorize another adult to obtain his/her Birth Certificate or that of his/her minor child (18 and under) by completing the Permission to Release Birth Certificate (English) or Permission to Release Birth Certificate (Spanish)  The Vital Records staff will NOT fax a Birth Certificate to anyone or any organization.
