Child and Adult Vaccinations
The Dubois County Health Department provides children and adult vaccinations to Dubois County, Indiana residents. The Dubois County Health Department Immunization Program operates in conjunction with the Indiana State Department of Health (ISDH). All immunizations are given according to the Center of Disease Control (CDC). For more info on vaccinations, Click Here.
We have the ability to bill through private insurance, after eligibility check is complete. Children who have Medicaid, no insurance, or are underinsured (meaning insurance without vaccine coverage), American Indian or Alaskan Native are eligible for vaccinations under our VFC program. The charge is $15.00 per administration.
If you are requesting a copy of your immunization record please print and complete our Immunization Release Record Form (English) or Immunization Release Record Form (Spanish).

Childhood Vaccinations
For children under 18 years of age, a parent or guardian must accompany them. If the parent or guardian is unable to attend the appointment, the child must bring (1) this form completed Parent Permission Form (English) or Parent permission Form (Spanish) (2) with the named person accompanying the child and (3) the named person must have an ID.
We will also need to see insurance card and a form of ID at the visit, if you are unable to provide these items, your appointment will be rescheduled.
